Parents often wonder if their child needs a regular yearly physical if they have already had a sports physical. Camp and sport physicals are different from a standard physical. A sports physical focuses on the health of your child as it relates to a specific sport or activity. It is more limited than a regular physical and more specific about athletic issues. In a regular physical, your provider will address issues unrelated to sports, focusing on growth and development, anticipatory guidance specific to your child’s age, and other preventative health issues. The provider will also ensure all vaccines are up to date. Please remember that abbreviated sport physicals do not take the place of yearly physicals as recommended by the American Academy of Pediatrics.
Concussion ImPACT Baseline Testing is recommended for all children over 6 years of age participating in contact sports. Please contact the office for more information.

We will gladly complete these sports physical forms for you. In order to do this, your child should have had a well child visit with one of our providers within the past 12 months. There is no charge for completing these forms, but please allow us at least 48 hours.
Most camps or schools provide the parent with specific forms to complete. Check your school or camp’s website to see if they provide a specific form. We do provide a standard UIL form (for grades 7th -12th) in case a parent doesn’t have one.
